I suspect each of them only drops with specific criteria, but what that is I am not sure. I mostly do strongholds in hard/challenging.For one thing, you only need to pick up all the confirmed kills when you're first building your stacks. Once you have full stacks, you only need to refresh. For another, the best Memento builds will use the kill confirmed buff in combat, because you'll get more awesome stuff like weapon damage and bonus armor whatnot.
In my experience no. I've been farming Summit daily for 2+ weeks for the Memento and I still don't have one. In my opinion freeroam is the best place to farm exotics. Every activity after completing it can drop a exotic. The activities are very quick to complete thus giving you a lot of drop chances per hour.
